I have had this problem several times before(every door). The regulator is cable operated and can come off the nylon pulleys. I have been able to get them back, but usually the nylon is damaged and it won't stay on. The other thing is the regulator is riveted to the door shell with 1/4" rivets. You will need a riveter capable of setting them. The best way to remove the rivets is to use a hammer and punch to first drive out the steel mandrel before drilling with 1/4" drill. The regulator itself is around $50 from Ford, you'll want to get the correct rivets at the same time. The rivets that hold the glass to the regulator are different than the ones that hold the regulator to the door(longer). You can get an inexpensive riveter at Harbor Freight for less than $20.
You have probably sheared some teeth off the regulater or the motor itself. Either way the door panel must come off to check it out. This can be a major job for the inexperienced.
