Could be that tension pulley is sticking from time to time. Try replacing it and see if that helps.
Nearly all such belts have 2 or even 3 associated bolts. One the tensioner, and 1 or more locking bolts.

Most obviously, if there is a locking bolt not undone so it releases the tensioner slider to move, the tensioner bolt can snap or strip because it is only designed to take the belt tension, no more, and is not a strong part.

Even if you are sure all the lock bolts are loosened, perhaps the slider parts are still binding tight.
