If the radiator is over 6+ years old then change it.

Make sure you only add 100% pure coolant into the reserve tank.

If the coolant level is low, this is exactly what will happen. Not enough coolant to circulate through the heater core, and the engine will run hot, and when it loses enough coolant, will overheat. Hope it's that simple. Let me know, if that's not it I'll help further.
